美国亚马逊产品被客户投诉有安全问题。现在产品已经下架。跪求各位大神下面是亚马逊发过来的表格

Product Safety Questionnaire

Purpose
To gather further information about a potential product safety concern.
General Questions
1	For the incident/concern mentioned in the e-mail, is this a known issue/complaint? If so, what actions have you taken to resolve the root cause of the issue in order to prevent future incidents?
2	Do you have any thoughts on what could have caused the issue?
3	What corrective actions are you planning to implement after you became aware of this incident/concern? What is the time frame for them to be implemented to your internal process?
4	Do you have a Product Safety or Product Quality Control system in place to identify potential product safety issues?
4.1	· If yes, please provide the detailed actions that are being taken within this system. In your opinion, why hasn't this particular issue been detected during your internal inspection process?
4.2	· If no, what steps do you normally take to ensure the safety of the products you sell?
5	To your knowledge, have any design changes been done to the product recently? If so, when was the product modified and what changes were implemented?
6	Has the product been part of a recall?

10	Manufacturer's address
11	What is the age group of intended user (e.g. below 3, 3-8, 8-12, 12-60, 60+, all)
12	How long has the brand been in existence? (<2 yrs, 2 -5 yrs, 5+ yrs)
13	How long has the product model been in existence? (<2 yrs, 2 -5 yrs, 5+ yrs)
14	How long have you been listing this product on Amazon website? (<2 yrs, 2 -5 yrs, 5+ yrs)
15	Is there an instruction manual and/or safety labels that come with the product? If yes, please attach the manual and images of safety labels in your response.
16	Does the product have US electrical safety certification? If so, can you provide testing documentation?
