各位大佬 符合性声明模板有吗？如何开通类目

Dear all

一个产品在欧洲站卖得好好的突然被查了，wu，厨具类的产品。

提交了各种资料，依然不通过，请问谁能给我一份符合性声明的模板？
另外关于产品图标，我拍了6个面的包装图给他，产品上有CE标，包装上没有，型号名称都可以贴上去，但是制造商名称包装上是没有的，这个也是不通过的原因吗？谁的产品包装上还印供应商的信息啊？

? Must include all sides of the packaging

If you are a re-seller/distributor, please provide us with at least 1 manufacture issued invoice (Invoice/Sales Invoice) that shows a purchase of 10 or more application related products. You will also need to provide one of the documents listed above.

The invoice you submit must meet the the requirements outlined below.

? Invoices must be dated within the last 180 days or reflect the delivery or purchase of products within the last 180 days.
? All documents must be valid and unexpired.
? All documents must be clear and legible. No information other than pricing may be redacted.
? All documents must include your name and address, as listed within your selling account.
? All documents must include the issuer’s name and contact information (e.g., phone number, address, e-mail, website, etc.).

Please provide us with the requested information within the next 14 days. If we do not hear from you in that time, this case will close and you will need to submit a new application.

我也是啊 一直提交一直说不符合要求 怎么搞？

下半年旺季，灯串类的 装饰这些 ，很难通过的，应该是亚马逊限制了，我也是提交了很多次。符合性声明一直说有问题

楼主 DOC通过了吗？求指导，还有欧盟的进口商信息怎么弄啊
